GOALS Medication Administration Instructions Form

Any youth participant bringing medications on their GOALS expedition - including medications prescribed by a medical professional and those purchased over the counter - must download, print and complete the INSTRUCTIONS FOR ADMINISTRATION OF MEDICINES FORM (link).

A separate form must be completed for each medicine taken a participant. Once completed, this form should be delivered to a GOALS leader at drop-off.
